How we support the transition?
We’re here to make the process simple and supportive for both you and your loved one:
- Initial Care Consultation: We begin with an initial consultation with our Family Liaison Managers, who will listen to your needs, answer your questions, and provide detailed information about our services.
- Show Round and Meet the Care Team: We schedule a visit to show you our facilities, meet our dedicated care team, and experience the warm, welcoming environment firsthand.
- Personalised Care Planning: Our care team will conduct a thorough assessment to develop a personalised care plan that addresses the specific needs and preferences of your loved one.
- Check-In on Preferred Date: Once the care plan is in place, we will arrange for your loved one to check in on a preferred date, ensuring a seamless and comfortable transition into our community.
